What Is a Routine EEG?

A routine electroencephalogram is a noninvasive test that records electrical activity from the brain using electrodes placed on the scalp.

The electrodes detect small electrical signals generated by brain cells. These signals are displayed as EEG waveforms that can be reviewed and interpreted by a physician trained in clinical neurophysiology.

A routine EEG commonly includes a relatively short period of recording. The total appointment usually takes longer because time is also needed for preparation, electrode placement, testing procedures and removal of the electrodes.

How Long Does an Ambulatory EEG Record?

Ambulatory EEG studies commonly record for 24, 48 or 72 hours. Some systems can record for 96 hours or longer. The duration selected depends on the clinical question, how frequently symptoms occur and the physician’s judgment.

The purpose of the longer recording is not simply to collect more data. It is to create a larger opportunity to record intermittent abnormalities, natural sleep or a patient’s typical episode.

The answer depends on what is meant by “accurate.”

An ambulatory EEG records substantially more brain activity than a routine EEG, which can increase the opportunity to detect abnormalities that occur intermittently. This can make ambulatory monitoring more sensitive in certain clinical situations.

However, ambulatory EEG should not be described as universally more accurate for every patient or neurological condition. Routine EEG and ambulatory EEG serve different roles, and diagnostic performance varies according to the patient population and the question being investigated.

What Does the Research Show?

A 2023 prospective study published in Neurology: Clinical Practice directly compared routine EEG and 24-hour ambulatory EEG in 100 adults who had experienced a first single unprovoked seizure.

In that specific group of patients, ambulatory EEG detected interictal epileptiform discharges or seizures with a sensitivity of 72%. Sensitivity was 11% for the first routine EEG and 22% for a second routine EEG.

The researchers found that 24-hour ambulatory EEG had greater sensitivity for detecting epileptiform discharges or seizures in this population.

These numbers should not be applied to every person undergoing EEG testing. The study involved adults being evaluated after a first unprovoked seizure, and the performance of EEG testing can differ in other clinical situations.

Why Can a Routine EEG Be Normal Even If Someone Has Epilepsy?

Epileptiform activity can be intermittent. A person may have epilepsy without showing an epileptiform discharge during the brief period covered by a routine EEG.

This is one reason neurologists do not use a routine EEG result by itself to diagnose or exclude epilepsy.

The physician also considers:

  • The patient’s description of the episode
  • Witness accounts
  • Medical and neurological history
  • Neurological examination
  • Imaging when appropriate
  • Other diagnostic testing

If a routine EEG is normal but clinical concern remains, additional EEG testing may sometimes be appropriate.

Why Does Recording Time Matter?

An EEG can only show what occurs while brain activity is being recorded. Increasing the recording period provides a larger sample.

A prospective study of 1,803 outpatient EEG recordings compared findings during the first 30 minutes with findings that appeared later in recordings lasting approximately one hour.

Among 426 patients who had interictal epileptiform discharges during their EEG, 81 patients, or 19.1%, had their first such discharge only after the initial 30 minutes. Longer recording also increased the rate of clinical event capture.

This research examined extended outpatient EEG rather than multi-day ambulatory EEG, but it supports an important principle: clinically useful EEG abnormalities may not appear during the earliest part of a recording.

What Does Diagnostic Yield Mean in EEG Testing?

Diagnostic yield refers to how often a test provides information that helps answer the clinical question being investigated.

In EEG testing, useful information can include:

  • Interictal epileptiform discharges
  • An electrographic seizure
  • A patient’s typical episode
  • EEG changes occurring during a reported event
  • Information that helps the physician characterize recurrent episodes

Diagnostic yield is not simply the percentage of EEG studies that are “abnormal.” Sometimes recording a patient’s typical event without an epileptic EEG correlate can also provide clinically useful information.

Why Can Ambulatory EEG Provide Additional Information?

Several features of ambulatory monitoring may help answer questions left unresolved by a short EEG.

A Longer Recording Window
